I have been given this case to solve in an attempt to understand the topic of patient clinical data analysis to develop my competency in reading and comprehending clinical data including, history, clinical findings, investigations, and come up with a diagnosis and treatment plan. 


CASE SCENARIO


     A 55 yr old male came to casuality with cheif complaints of fever since 6 days and inability to walk since 4 days.


HISTORY OF PRESENTING ILLNESS


Taken to local hospital 2 days ago. Found to have GRBS around 30 and ketone bodies found in urine.

Low grade intermittent fever since 5-6 days

Generalized body weakness and unable to walk since 4 days .


Was given treatment there and brought to our hospital for further treatment.


No other complaints

Type 2 DM present

    Not a k/c/o Asthma, Epilepsy, TB, HTN



TREATMENT HISTORY 

Taking OHA for diabetes since 7yrs

Occupation - farmer

Micturition- normal

 No known allergies

Addictions

Occasionally consumes alcohol and smokes tobbaco since 20 years.
